Island Harvest is a proud provider of Medicaid Enhanced Nutrition Services which supports eligible Medicaid members by providing access to screenings, navigation, and access to housing, nutrition, and transportation services. 

Island Harvest has joined the Health Equity Alliance of Long Island (HEALI) to provide these services at no cost to members who screen positive for an unmet health-related social need.

What is provided in the Medicaid Enhanced Nutrition Services?

Upon completion of a screening and eligibility assessment, Medicaid Managed Care members residing in Nassau or Suffolk Counties may be eligible for:

  • Nutrition screening and navigation
  • Pantry stocking / grocery boxes
  • Nutritionally appropriate food prescription boxes
  • Individualized nutrition counseling and group classes
  • Enhanced care management
